Due to recent federal legislation (the Paperwork Burden Reduction Act), DFC Company is no longer required to automatically mail a paper Form 1095-C to all employees.

What is Form 1095-C?

This form provides information about the health insurance coverage offered to you and your family. While it may be helpful for your records, you do not need a copy of this form to file your federal or state taxes.

How to Request a Copy of Form 1095-C

If you would like to receive a copy of your Form 1095-C, you may request one at any time. We will provide the form to you within 30 days of receiving your request.
